Ball-of-Foot Pain Buying Guide

The ball of the foot is the weight-bearing section of the forefoot immediately before the toes. It takes a considerable share of your body weight as you move forwards through each step. This part of the foot carries considerable pressure during walking, running and other weight-bearing activities.

Pain here can have several causes such as exercise, tight footwear and foot shape, as well as conditions including Morton’s neuroma and bunions.

Our Ball-of-Foot Collection covers metatarsalgia, Morton’s neuroma, sesamoiditis and stress fractures. Although these conditions can affect a similar part of the foot, the symptoms and underlying problem can be quite different.

Common Causes of Ball-of-Foot Pain

Metatarsalgia

Metatarsalgia describes discomfort affecting the forefoot rather than identifying one single underlying condition. The pain can be spread across the area behind the toes and may become more noticeable as time spent walking or standing increases.

Symptoms may include aching, tenderness, burning or discomfort that becomes more noticeable during walking, standing or exercise.

Pressure from activity, footwear or the way weight is distributed across the forefoot can all contribute.

Our Metatarsalgia collection includes metatarsal pads, insoles, cushioning and other products intended to reduce pressure and improve forefoot comfort.

This collection may be relevant if:

  • Your discomfort is spread across the ball of the foot
  • Walking or standing increases forefoot pain
  • The area feels tender or overloaded
  • You want cushioning or broader pressure relief
  • There is no obvious nerve-related or highly localised bone pain

Morton’s Neuroma

Morton’s neuroma is a nerve-related forefoot problem. Rather than producing only an ache from pressure, it can create sharper or unusual sensations that may travel towards the toes.

Some people experience electrical, hot or stabbing discomfort, while others notice altered sensation in the neighbouring toes. A feeling that something is trapped or folded beneath the front of the foot is also commonly reported.

Our Morton’s Neuroma collection includes metatarsal cushioning, insoles and other products aimed at reducing pressure around the forefoot.

This collection may be relevant if:

  • Pain feels burning, shooting or nerve-like
  • You experience tingling or numbness around the toes
  • It feels as though there is a small stone beneath your forefoot
  • Tight or narrow shoes make symptoms worse
  • Discomfort is concentrated between the metatarsal area and toes

Sesamoiditis

Sesamoiditis produces a more concentrated pattern of pain around the inner side of the forefoot. The small sesamoid bones lie within the structures that help the big toe work during push-off, so activities that repeatedly load this part of the foot can make the area sensitive.

Symptoms are often most noticeable when transferring weight forwards through the big toe rather than being spread evenly across the whole forefoot.

Our Sesamoiditis collection focuses on cushioning and offloading the painful area through products such as metatarsal pads, insoles and protective padding.

This collection may be relevant if:

  • Pain is directly underneath the big-toe joint
  • Pushing through the big toe aggravates symptoms
  • The area is particularly tender when weight-bearing
  • Running or repetitive forefoot loading has contributed
  • You want to reduce pressure around the sesamoid region

Stress Fractures

Stress fractures are overuse injuries in which repeated impact gradually overwhelms a bone's ability to recover. When a metatarsal is affected, the resulting discomfort can initially resemble other forms of forefoot pain.

A useful distinction is that stress-fracture pain is often focused around a relatively small area and becomes increasingly noticeable when the foot is repeatedly loaded.

Our Stress Fractures collection contains cushioning, insoles, metatarsal pads and other supports that may be useful during suitable stages of recovery.

A suspected stress fracture should be professionally assessed, as cushioning products are not a substitute for any necessary offloading or medical treatment.

Consider professional assessment for a stress fracture if:

  • Pain began after repeated running, jumping or increased activity
  • One specific area of the foot is very tender
  • Weight-bearing progressively increases the pain
  • Swelling has developed
  • You suspect a bone injury rather than general forefoot discomfort

Which Ball-of-Foot Condition Could Be Causing My Pain

Symptoms can overlap, so this should be used as a general guide rather than a diagnosis.

Our main symptom pattern | Explore

A broader ache or tenderness across the front of the foot | Metatarsalgia

Sharp, hot or unusual sensations that may extend towards the toes | Morton’s Neuroma

Pain centred around the underside of the big-toe area | Sesamoiditis

A small, clearly defined painful area that worsens with repeated impact | Stress Fractures

Persistent forefoot pain is worth assessing properly if the cause is uncertain. The NHS specifically advises against trying to diagnose ball-of-foot pain yourself because several different conditions can produce similar symptoms.

What Products Can Help With Ball-of-Foot Pain?

The most appropriate support depends on the cause and exact location of your discomfort.

Metatarsal Pads & Forefoot Cushions

Metatarsal pads and forefoot cushions add support around the area behind the toes. Depending on their shape and position, they can take some of the load away from an uncomfortable part of the forefoot and make standing or walking feel less harsh.

Different designs include gel pads, fabric sleeves and adhesive cushioning. Our Ball-of-Foot collection currently includes several metatarsal cushioning products.

They may be particularly relevant for metatarsalgia, Morton’s neuroma and sesamoid-related pressure, although positioning varies depending on the condition.

Padding should make the foot more comfortable rather than creating a raised pressure point directly beneath the painful area.

Supportive Insoles

Insoles are worth considering when the problem is not confined to one small area. By supporting more of the sole, they can combine forefoot cushioning with support beneath the arch and heel.

Depending on the design, an insole may provide:

  • Forefoot cushioning
  • Arch support
  • Heel stability
  • Shock absorption
  • More even pressure distribution

Make sure there is enough space inside the footwear after fitting an insole. Reducing room around an already painful forefoot can make symptoms worse.

Supportive Footwear

Footwear can have a substantial effect on forefoot pressure.

Shoes that are narrow, pointed or excessively tight can compress the front of the foot, while high heels move more body weight towards the forefoot. We recommend comfortable footwear with enough room and suitable cushioning as part of self-care for ball-of-foot pain.

Look for:

  • Enough width across the forefoot
  • Adequate room for the toes
  • Underfoot cushioning
  • A secure fit
  • A stable sole
  • Enough internal space for pads or insoles

Cold Therapy

Cold therapy can provide temporary relief when the forefoot feels particularly sore following activity.

Wrap a cold pack before placing it against the skin and use it for short periods. It should be viewed as a way to manage symptoms rather than a solution to the underlying cause of the pain.

Foot Rollers & Massage Products

Foot rollers and massage products can be useful for general relaxation of tired feet. The wider Ball of Foot collection currently includes a manual foot roller alongside more targeted cushioning products.

Be cautious with direct pressure over a painful nerve, sesamoid or suspected bone injury. Massage should feel comfortable rather than reproduce sharp or increasing pain.

How to Choose the Right Ball-of-Foot Support

  • If Pain Is Spread Across the Forefoot – Start by considering metatarsalgia products, particularly metatarsal cushioning or supportive insoles. The aim is generally to reduce concentrated pressure through the ball of the foot.
  • If Symptoms Feel Burning or Tingling – Consider whether the symptoms are more consistent with Morton’s neuroma. A wider shoe and correctly positioned metatarsal support may be more relevant than simply adding cushioning directly beneath the painful area.
  • If Pain Is Under the Big Toe – Look at the Sesamoiditis collection and prioritise offloading the area beneath the big-toe joint. Avoid adding a thick pressure point directly underneath the sore sesamoid region.
  • If Pain Is Highly Localised After Exercise – A stress fracture should be considered, particularly following a sudden increase in running, jumping or other repetitive activity. Do not rely on a pad or insole to continue through progressively worsening bone pain.

Looking After a Painful Forefoot

The correct approach depends on the underlying cause, but general measures may include:

  • Reducing activities that repeatedly aggravate the pain
  • Wearing wider, cushioned footwear
  • Using appropriate pads or insoles
  • Avoiding high heels while symptoms are active
  • Applying a wrapped cold pack where appropriate
  • Returning to higher-impact activity gradually
  • Seeking assessment if symptoms persist or worsen

For uncomplicated ball-of-foot pain, we recommend measures including rest, suitable footwear, soft insoles or pads and avoiding activities that make symptoms worse.

Frequently Asked Questions About Ball-of-Foot Pain

What is the ball of the foot?

The ball of the foot is the padded forefoot area immediately behind the toes, around the metatarsal heads and associated joints.

It carries substantial load as your body moves forwards during walking and running.

What causes pain in the ball of the foot?

There are many possible causes.

Common possibilities include metatarsalgia, Morton’s neuroma, sesamoiditis, repetitive activity, unsuitable footwear and injuries affecting the metatarsal bones.

The location and type of pain can provide clues, but persistent symptoms should not be diagnosed from symptoms alone.

Is ball-of-foot pain the same as metatarsalgia?

Metatarsalgia is the general medical term commonly used for pain around the ball of the foot.

However, the underlying reason for that pain can vary. More specific conditions such as Morton’s neuroma or sesamoiditis can also produce discomfort in the forefoot.

How can I tell Morton’s neuroma from general metatarsalgia?

General metatarsalgia commonly produces aching or tenderness through the forefoot.

Morton’s neuroma is more likely to involve burning or shooting pain, tingling, numbness or the sensation of something underneath the foot, particularly towards the toes.

Can sesamoiditis cause ball-of-foot pain?

Yes. The sesamoid bones sit beneath the joint at the base of the big toe, which forms part of the forefoot. Sesamoiditis therefore tends to produce ball-of-foot pain that is specifically concentrated around this area.

Can a stress fracture cause ball-of-foot pain?

Yes, particularly when a metatarsal bone is affected.

Stress-fracture pain is often relatively localised and becomes more noticeable during repeated weight-bearing.

Are metatarsal pads good for ball-of-foot pain?

They may help in appropriate circumstances by cushioning or supporting the metatarsal region and changing how pressure is distributed.

The position of the pad matters, and the ideal placement can differ depending on whether the problem is metatarsalgia, Morton’s neuroma or another forefoot condition.

When should I seek professional advice?

Consider professional assessment if the pain is worsening, repeatedly returning, and affecting normal activities or has not improved after around two weeks of self-care.

Seek more urgent advice if the pain is severe, you cannot walk, the foot changes shape, becomes hot and swollen, or the symptoms followed an injury where a fracture may be possible.
